Title: Rehabilitation Support Worker
Reports to: Manager

SUMMARY

The Rehabilitation Support Worker’s primary function is to assist clients with achieving their optimal level of independence. Under the direction of a regulated healthcare professional, the Rehabilitation Support Worker provides support services to clients living with the effects of an acquired brain injury and/or mental health diagnosis. Programs that may be implemented include life skills retraining, behavioural management, cognitive remediation, community reintegration, educational and vocational support.

RESPONSIBILITIES

  • Effectively and professionally implement treatment programs under the direction of the rehabilitation team.
  • Meet with clients as specified by the treatment team and as set out in the treatment plan.
  • Implement therapy goals into the client’s everyday routines in a way that ensures that the client modifies activities of daily living in a consistent, structured and routine fashion.
  • Communicate (both written and verbally) to regulated healthcare professionals regarding client progress and intervention strategies.
  • Provide input into treatment objectives and client’s programming when requested.
  • Liaise with all members of the treatment team to ensure effective service provision and attend team meetings as scheduled.
  • Review all documentation provided by members of the treatment team.
  • Communicate with Manager on a regular basis, seeking guidance when necessary.
  • Prepare and send detailed progress notes to Manager for each client on a weekly basis, as specified.
  • Prepare and send progress reports to Manager for each client on a regular basis, as specified.
  • Participate in mandatory ongoing training and professional development.
  • Conduct self in professional and courteous manner at all times.
  • Exercise good personal time management and consistent punctuality.
  • Treat every client with dignity and respect.

EDUCATION & EXPERIENCE

  • Relevant post-secondary education within the health or social services field.
  • Minimum four years experience in acquired brain injury and/or mental health.
  • In-depth knowledge and experience with the design, development and implementation of behavioural modification strategies and interventions.
  • Demonstrated excellent interpersonal and communication skills.
  • Personal computer skills including familiarity with Microsoft Office software.

REQUIREMENTS

  • Maintain up-to-date criminal reference checks including vulnerable sector screening.
  • Valid driver's license and clear driver's abstract.
  • Maintain certification in Non-Violent Crisis Prevention and Intervention.
  • Updated certification in CPR and First Aid.
