Genesis Community Rehabilitation, Inc. accepts referrals from many sources including Physicians, Case Managers, Occupational Therapists, Physiotherapists, Speech Language Pathologists, inpatient programs, families, schools, rehabilitation consultants, insurance companies and legal representatives.

Services are provided to individuals from children to adults, inclusive, residing in the South Central region of Ontario and living with the effects of an acquired brain injury and/or mental health diagnosis.

• A list of suitable Rehabilitation Support Workers (RSWs) and their curriculum vitaes will be provided to the referral source. Attention is focused on ensuring the RSW’s approach, skills and location best suit the needs of the specified client.
• Once a suitable match is established, a treatment plan is completed, signed by a supervising regulated healthcare professional and submitted to the insurer.
• Any relevant documentation such as medical reports, progress reports, assessments, etc. should be faxed to 905-633-9001.
• Once approval is obtained from the insurance company, the RSW will contact the supervising healthcare professional and coordinate a meeting to be introduced to the client.
• Once support services are initiated, weekly progress notes and bi-monthly progress reports are submitted to all members of the Treatment Team.
